Changes to registration during Covid-19 Pandemic

Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete our online registration form.

 

New Patient Registration

Church Lane Surgery accepts patients from Brenzett, Greatstone, Ivychurch, Littlestone, Lydd on Sea, New Romney, Old Romney and St Marys Bay.

 

Registration

Registering is easy, simply fill out our online form, scan your ID and submit it all securely to the surgery

Please be assured that all ID documents will be destroyed securely in line with GDPR and NHS Guidelines. 

If you do not have online access you will be able to collect a registration pack from our reception team to take home to fill out and return to the surgery with the necessary ID documentation. 

Please note that patients are registered with the practice and not with a particular doctor. You will however be offered a named doctor who will look after you and this will be our Lead GP.

 

Moving

If you move outside of the surgery area, you will need to register with another surgery within your new catchment area.

 

Changes in Personal Details

Please notify the surgery as soon as possible of any change of name or address by completing this online form. Could you please include your postcode and telephone/contact number.  In an emergency this may be vital. 

If you are ex-directory and are concerned about divulging your telephone number, we can assure you that this is required for surgery purposes only. 

Medical treatment is available from the date of registration. Please contact reception for further information.

Online Patient Access

This Surgery allow patients to book appointments and order repeat medication online. To be able to use these features please click here and register for Online Patient Access.

 

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
